SENIOR SCHOLARSHIP REPORT FORM
Please complete the scholarship report form to aid us in reporting all scholarships our students receive to WCPSS. Include all scholarships and monetary awards you have been offered, even if you will not accept the offer. Financial aid should not be reported. Copies of the awards letters are required for a form to be completely processed. Scholarship recipients will be announced (pending student approval) during the Senior Assembly in the spring.

Counselor Statement/Counselor Recommedation Request
This form is used to record all counselor recommendations to be processed by Student Services. We ask that you allow 10 business days for processing. Please provide Student Services with stamped, addressed envelopes when asking for recommendations.

Co-Enrollment Form
Students requesting to earn WCPSS graduation credit through a co-enrollment opportunity must first meet with his/her School Counselor to discuss the request and determine eligibility. Once eligibility is determined, the student, along with his/her parent/guardian, must submit a signed WCPSS Co-Enrollment Form to his/her school counselor for Principal approval.
